Preceded in death by parents, Joseph and Mary; brother, Joseph; sister, Rita; former wife, Gloria Kotrba. Survived by daughter and son-in-law, JoAnn and David Soukup; son, John Kotrba; four grandchildren; brother and sister-in-law, Stanley and Joan Kotrba, Wichita, Kan.; sisters and brother-in-law, Mary Ann Curtiss, Alliance, Ludmilla and Victor Stamos, Minneapolis, Minn.; nieces; nephews.

Survived by sons and daughters-in-law, Rob and Crystal Blum, Wesley Blum, and Jeremy and Lauren Blum all of Bellevue; father, Robert Sullivan, Bellevue; sister and brother-in-law, Barb and Jim Abraham; brothers and sisters-in-law, Dennis and Lisa Sullivan, and Mike and Stephanie Sullivan, Houston, Texas; grandson, Trever Blum.

Preceded in death by parents, Edward and Josephine Janousek; son, Jeffrey; brother, Edward. Survived by wife, Phyllis; daughters and sons-in-law, Michelle and Rick Vlcek, Jill and Jim Grob, and JoLene and Nathan Bartels, Elk Creek; sons and daughters-in-law, Brian Todd and Diana Janousek, Des Moines, Iowa, and Kurt and Kim Janousek; 12 grandchildren; two great-granddaughters.
